A review of a Fullers' ferry collision that injured 19 people in Auckland has found the boat's master wasn't properly trained.
It's also found the company knew there was a fault in a newly-installed berthing system.
The ferry suffered significant damage to its hull when it struck the Devonport Wharf in 2015.
No warning was given to passengers to brace, and bench seats toppled forward, injuring 19 people.

Fullers has already been fined $140,000 in the Auckland District Court for safety failings.
